• Beneficiary Designation Form for Vehicle Title Transer Upon Death is available online at the MVD’s website, as form 96-0561 (“Beneficiary Designation” see page 3).
• How to Check Your Property Title
  • Go to http://mcassessor.maricopa.gov/assessor/
  • If you know the Parcel Number of your property, type that in the boxes upper right and click Go
  • If you do not know your Parcel Number, click on Advance Search Options.
    
    
  • Type in your street name and number without North, West, Drive, Lane, etc.
    
    
  • Click on Deed# under the Owner Information section. This will take you to the Maricopa County Recorder website.
    
    
  • Click on #1 under Pages to see a copy of your deed.
